After the 12 hand seals are preformed, the user puts their chakra into a seal, this seal then releases a physical form of chakra that appears to be arms. There are many of these arms, and they fly at the target at an alarming speed, when they reach the target they break down the targets chakra at a quick pace, and seals the broken down chakra into a new seal onto the users hand, this seal needs to be put into a medium within a four post time period. The chakra from this seal can be transferred by the jutsu/seal below.

The user does two things, one simple and one more complex. The user transfers the previously used Sho Kujo Hekai no Itonami Fuuin no Jutsu to get rid of the chakra on the users hand or from the medium that the previous seal had been put into the store the chakra. They then put it into a human host, such as a tailed beast with a jinchuriki. Now here is the complex part, the user puts part of their chakra into the target host and are able to activate the second process of this jutsu. Because the result of this jutsu, the 'host', is connected to the user, the user can sense when the user has an enormous spike or drop of chakra power (such as if a tailed beast is giving the target too much power or taking away too much); so then the user of this jutsu can activate a mini Sho Kujo Hekai no Itonami Fuuin no Jutsu where the seal was placed that eats away at the excess powerful chakra or places more chakra into the target, whichever the user decides to do. The user can also summon themselves through this jutsu, if they have Kuchiyose no Jutsu, but this isn't a requirement, just a smart choice.
